Please complete the form below to update us on what you would like to do with the funds you paid for Spring 2020 Season that was impacted by COVID-19 closures.

 

ALL credits must be refunded or in the process of being utilized by October 31, 2020 or it will be considered a donation to Mo’ Motion, Inc., a 501(c)3 non-profit organization, and allocate those funds towards scholarship players or other outreach programs in New York City.
